At least 6 million Americans suffer from panic attacks and panic disorder both conditions classified as anxiety disorders. According to the Anxiety and Depression Association of America (ADAA), about 2-3% of Americans experience panic disorder in a given year and it is twice as common in women as in men. Panic disorder typically affects individuals when they’re in their 20s but is also seen in young children, adolescents, and older adults.
Cognitive behavioral therapy (CBT), is based on the idea that our thoughts cause our feelings and behaviors, not external things, like people, situations, and events. According to the National Association of Cognitive Behavioral Therapists the benefit of this therapy is that we can change the way we think to feel and act better even if the situation does not change. CBT focuses on determining the thought and behavior patterns responsible for sustaining or causing the panic attacks. CBT is a time-limited process (treatment goals—and the number of sessions expected to achieve them—are established at the start) that employs a variety of cognitive and behavioral techniques to affect change.
Anxiety disorders often first appear in childhood. This is a very good time to intervene or seek treatment, because children's brains are still developing, and can more easily adapt to new "modes" of thinking, relative to adult brains. Helping your child cope with an anxiety disorder can be a complex task, potentially involving family members, friends, teachers and counselors, and mental health professionals. It's important to note that everyone feels anxiety to some degree regularly throughout their life - fear and anxiety are adaptive and helpful emotions that can function to help us notice danger or threat, keep us safe, and help us adapt to the environment. Anxiety disorders represent states when fear or anxiety becomes severe or extreme, to the extent that it causes an individual significant distress, or impairs their ability to function in important facets of life such as work, school, or relationships. It is also important that risk factors don't at all imply that anxiety is anyone's fault; anxiety disorders are a very common difficulty that people experience. In this section, we will review risk factors for anxiety disorders. There are many potential risk factors for anxiety disorders, and most people likely experience multiple different combinations of risk factors, such as neurobiological factors, genetic markers, environmental factors, and life experiences. However, we do not yet fully understand what causes some people to have anxiety disorders.
Mitral valve prolapse (MVP), also called "click murmur syndrome" and "Barlow's syndrome," is the most common type of heart valve abnormality. Usually, people with mitral valve prolapse have no signs and symptoms; however, if the prolapsed valve is severe, symptoms may appear. When symptoms of severe mitral valve prolapse do appear, they may include, fatigue, palpitations, chest pain, anxiety, migraine headaches, and pulmonary edema. Echocardiography is the most useful test for mitral valve prolapse. Most people with mitral valve need no treatment. However, if the valve prolapse is severe, treatment medications or surgery may be necessary to repair the heart valve.
Neural circuitry involving the amygdala (which regulates emotions like anxiety and fear, stimulating the HPA Axis and sympathetic nervous system) and hippocampus (which is implicated in emotional memory along with the amygdala) is thought to underlie anxiety. People who have anxiety tend to show high activity in response to emotional stimuli in the amygdala. Some writers believe that excessive anxiety can lead to an overpotentiation of the limbic system (which includes the amygdala and nucleus accumbens), giving increased future anxiety, but this does not appear to have been proven.
In contrast, the term anxiety attack is not a specifier outlined in the DSM-5. Rather, anxiety is used to describe a core feature of multiple different anxiety disorders. The culmination of symptoms that result from being in a state of anxiety—such as restlessness, shortness of breath, increased heart rate, and difficulty concentrating—may feel like an “attack,” but are generally less intense than those experienced at the height of a panic attack.
Medication can be used to control or lessen symptoms related to panic disorder. It is most effective when combined with other treatments, such as the aforementioned cognitive behavioral therapy and exposure therapy. Medications used to treat panic attacks and panic disorder include antidepressants, though they take several weeks to reach effectiveness. Benzodiazepines such as Ativan and Xanax work quickly. However they are addictive and should only be used for a short time,
Secondly, the psychobiological conceptualization of panic disorder emphasizes the influence of psychological factors (Meuret, White, Ritz, Roth, Hofmann, & Brown, 2006). This psychological factor refers to a fear of bodily sensations, or a certain set of beliefs that lead individuals to be especially afraid of physical symptoms, such as believing that a racing heart could mean heart disease. Sometimes this is discussed as anxiety sensitivity or a belief that anxiety is harmful. Again, having the belief that physical symptoms are harmful may increase the likelihood of experiencing a panic attack, but it does not make having a panic attack inevitable. Instead, panic attacks can seem abnormal if they occur at the wrong time, when there is no real reason to be afraid. It is important to consider, however, that anxiety can also be adaptive or helpful in contexts where there is true threat.
One of the scariest early experiences in panic disorder is having a panic attack and not knowing what is happening to your body. By learning more about panic attacks and panic disorder, you can start to label and identify the experience that you are having. Although the experience of panic attacks is very distressing, having a panic attack will not cause you to die or to completely lose control and they do not mean that you are going crazy. Sometimes, just knowing what is going on can help people to feel better. For example, the next time you have a panic attack, you can tell yourself "this is anxiety. I have felt this before and I was okay."

My dad passed away in November and I went back to work after a month, a month later I was given another client to work on. I felt really stressed out. I found myself feeling irrationally angry about things at work, build things up in my head to be really bad and then I would need to cry to release it, I have had two recent experiences of what I think are panic/anxiety attacks- feeling overwhelmed/stressed/negative thoughts and then hyperventilating with non-stop crying. I am seeing a psychiatrist who I think is helping. But short-term I think I need to tell work about how I am feeling. I want to quit and have time off but get worried about money.
Biological causes may include obsessive compulsive disorder, Postural Orthostatic Tachycardia Syndrome, post traumatic stress disorder, hypoglycemia, hyperthyroidism, Wilson's disease, mitral valve prolapse, pheochromocytoma, and inner ear disturbances (labyrinthitis). Dysregulation of the norepinephrine system in the locus ceruleus, an area of the brain stem, has been linked to panic attacks.
Social risk factors for anxiety include a history of trauma (e.g., physical, sexual or emotional abuse or assault), early life experiences and parenting factors (e.g., rejection, lack of warmth, high hostility, harsh discipline, high parental negative affect, anxious childrearing, modelling of dysfunctional and drug-abusing behaviour, discouragement of emotions, poor socialization, poor attachment, and child abuse and neglect), cultural factors (e.g., stoic families/cultures, persecuted minorities including the disabled), and socioeconomics (e.g., uneducated, unemployed, impoverished although developed countries have higher rates of anxiety disorders than developing countries).
Here’s another way to differentiate those feelings. Let’s say you’re someone who suffers from panic attacks. If you’re worried about having the next panic attack, that’s “anticipatory anxiety—the worry of, Oh, no, I’m going to have a panic attack,” says Brown. On the other hand, “The emotion I’m feeling when I’m having a panic attack we would think of as fear.”
Cognitive therapy and exposure therapy are two CBT methods that are often used, together or by themselves, to treat social anxiety disorder. Cognitive therapy focuses on identifying, challenging, and then neutralizing unhelpful or distorted thoughts underlying anxiety disorders. Exposure therapy focuses on confronting the fears underlying an anxiety disorder to help people engage in activities they have been avoiding. Exposure therapy is sometimes used along with relaxation exercises and/or imagery.
